Role of nurse in organ donation in pediatric patiens

The topic of my diploma thesis is the issue of nursing care in pediatric patients before consumption of organs at the resuscitation department. I consider this topic socially very topical because the number of transplant candidates on transplantation lists is constantly increasing. Since child transplant candidates are the most vulnerable group, I have focused on child organ donors. The theoretical part criticizes the concept of death and threat, including examination methods that lead to brain death. I present the activities of the Coordination Center of the Transplant and Coordi- nated Coordinator in a donor program that seeks to proliferate the transplant program. An important component is legal legislation and ethical aspects, which are an integral part of the donor system in the Czech Republic. Study materials I searched in the databases NLK, SVKKL, Medvik, Embase, PubMed, Wiley Online Library, Medscape. The research was conducted from the National Medical Library and from the Central Bohemian Library in Kladno. I got statistical data from web sources, especially from KST. Medical Specialist Awareness and Attitude about Organ Donation

Bachelor thesis is focused on finding the awareness and attitudes of nurses towards organ donation. The work is divided into two parts ̶ theoretical and empirical. In the theoretical part summarizes available data on the history and development in this field, applicable legislation is mentioned, care for organ donors and overview of the current situation of organ donation, along with current trends. Empirical part took the form of quantitative research, which was conducted by anonymous questionnaires. The aim was to determine nurses' awareness about organ donation organization, determine their orientation in the field of diagnosis of brain death, to map the contemporary practice in the care of a dead donor organs and detect personal attitude on the issue of donation. Respondents were nurses that during their practice on the wards encounter this issue. Questionnaires were distributed on anesthesiology and resuscitation department of Central Military Hospital ̶ Military University Hospital in Prague, then to the operating room and intensive inpatient department of intensive and resuscitation care at the Institute for Clinical and Experimental Medicine. Nursing care for cadaverous patients and nurses views on organ donation.

This Bachelor degree work is focused on the issue of the nursing care for the cadaverous patient, on its specific features, on the knowledge of nurses and on their opinions of the transplantation program. Result of the work creates an information brochure which will serve as a guideline for all nurses of the Department of Anaesthesiology and Resuscitation in Písek Hospital and it will help them to be quickly and well informed on the nursing care for the donors. The work is divided into two parts. In the first part there are stated legal regulations which run the transplantation process in the Czech Republic, next there are mentioned duties of the doctors from the donor hospital and examinations which are necessary for pronouncing the brain dead. The first part also describes activities of the coordination centre which is in charge of the process itself. Further, there is dealt with the ethic of the organ transplantation. The description of the nursing care for the donor with the beating heart forms a major part of the work. It involves the basic nursing and intensive care, specific examinations and documentation, doctors´ assistance and patients´ monitoring. For the second research part there were set two aims, which we tried to explain by means of interviews involving several groups of questions. Respondents were only nurses who have already had experience with the care for donors. The research questions were answered by means of the qualitative research investigation using the form of the semi-structure interview. The aims were supposed to map out the nursing care for donors from nurses´ point of view and to map out the nurses´ opinion of the donorship. The groups of questions concerned the specific features of the nursing care, nurses´ knowledge in this care and their opinion of the transplantation program. The research confirmed that this care has its specific characteristics, nurses have sufficient knowledge about the issue and they have mostly positive opinion of transplantation. On the basis of the research there was compiled a brief information brochure designed not only for beginning nurses. Students of medical branches can benefit from the work too.
Ethical Aspects of Organ Transplantations

In the introduction the theses deals with change of medicine paradigma and shows development and changes of transplantation method. Then is discussed the classification of transplantation and presents general conditions and biological positions, which are connected with medical ethics of organ transplantations. There are presented (and discused) important ethical aspects of cadaveric transplantation, problems of live organ donors and question of xenotransplantation. Relevant International ethical codes and legislation in the Czech Republic is also mentioned. The analysis at the end shows the urgency to engage in the problems of extended posibilities and the need of taking some limits. It is also necessary to educate professional, responsible and empathic physicians.
